To ensure timely reimbursement, your information must be received as follows:

• Applications received within 30 days of course(s)/certification exam(s)/recertification(s) start date.

• All correct, applicable reimbursement documentation received within 90 days after course(s)/certification exams(s)/recertification(s) completion date.
